Step 1: The PRI
The PRI is an abbreviation for the Patient Review Instrument. This is a medical assessment conducted by a physician or nurse. There are several means of obtaining a PRI. In most instances, your physician can complete the PRI. Independent agencies such as Elder Plan also have qualified nurses to complete such forms for a nominal fee. If you are having difficulty obtaining a PRI, please contact our admissions department directly at ext. 326 or ext. 330.

Step 2: Fax the PRI to the admissions department at 718-467-7315
Once the PRI is completed, fax the document to our admissions department @ 718-467-7315. An admissions coordinator will contact you to inform you of acceptance or denial into the facility.

Step3: Personal and Medical Documentation
Our financial coordinators here at MGNH will process all financial business. In order to do so the following documentation will be needed:
Medicaid Card
Social Security Card
Birth/Baptism Certificate
Please provide one document from each of the following applicable groups:
Group 1
Naturalization Certificate
Alien Registration Card (Green Card)
United States Passport
Group 2
Marriage Certificate
Death Certificate of Spouse (or funeral bill)
Military Discharge papers
Group 3
Deed/Mortgage information
Proof of address (rent receipts preferred)
Latest utility bills (or letter from individual responsible for bills)\
Group 4
Latest pension check/letter of verification
Savings account bankbook (past 30-36 months)
Checking account statement (past 12-18 months)
Group 5
Stocks and Bonds
Life insurance policies

Step 4: Transportation to the facility
Family members generally make transportation arrangements. If you need assistance doing so, our facility works with several ambulance/ambulette companies, and can facilitate transportation (family members are responsible for all associated transportation fees).

Step 5: Arrival at the facility
Upon arrival at the facility, one of our admissions coordinators or the Director of Social Services will welcome you and your loved one. This person will also serve as an escort to the floor and room where your loved one will reside. Depending upon your loved one's social and medical needs, as well as availability, she/he will be placed in an appropriate room conducive with his or her needs.
